Bacterial Meningitis Vaccination Requirement
Any incoming new student must either receive a vaccination against bacterial meningitis or meet certain criteria for declining such a vaccination. For more informaiton please refer to the Student Health Services web page.
If you have not already received the meningococcal (meningitis) vaccine, contact your primary health-care provider or your local public health clinic to determine availability. Most large pediatrician offices offer the vaccine and the cost will depend on your insurance coverage. Most local health departments will offer the vaccine at a low cost. However, these offices offer limited vaccination times so be sure to call ahead. Vaccinations for bacterial meningitis cannot be older than 5 years or a booster will be required.
Where to Send Your Vaccination Records
Please send an official copy of your bacterial meningitis vacination records to our Director of Records and Registration Pamela Pipes by one of four methods:
Scanning and attaching the record to an email as a PDF to email@example.com.
Faxing to (432) 837-8411.
Dropping off the record at her office in Lawrence Hall room 213.
Mailing it to: Sul Ross State University, Center for Enrollment Management, Box C-2, Alpine, TX 79832.
Where to Send Your Exemption Affidavit or Medical Waiver Letter
Please send your completed affidavit (signed and notarized) via one of the following methods (Please note that affidavits cannot be emailed or faxed):
