Student Checklist

Greetings!  This is an updated checklist of "please do's" that will help make your enrollment experience as efficient and pleasant as possible.  Except where otherwise indicated, hardcopies of all required forms are available through Tomas Hernandez (tohernandez@midland.edu) and Brandi Bell (bbell@midland.edu) on the Midland College campus located in the Science Faculty Building.

FOR ENTRANCE TO THE PROGRAM:

1.  Complete the application for admission to SRSU (the common application, which can be completed online, is ideal).

2.  Complete the S.I. one-page form "Transcript Release".

3.  Complete the S.I. one-page form "Personal Information".

4.  Be sure to get and/or request transcripts for ALL previously attended universities and colleges.  Due to our special consortial agreement, transcript materials available at Midland College can be swapped immediately and without cost to SRSU.  If you have questions regarding what materials are currently held for you at Midland College, please consult Latisha Williams in the MC Registrar's Office (lwill@midland.edu).

FOR FINANCIAL ASSISTANCE:

1.  Complete the FAFSA (can be done online).

2.  For each term in which the student would like to apply for financial aid and scholarships, the short Consortial Agreement for Financial Assistance must be completed.  This is a document that circumvents our offices, so please inform a Science Initiative agent when this is completed so that we may best advocate on your behalf.

3.  Complete the SRSU Scholarship application (available online).

4.  Any specific questions about ideal arrangements for financial assistance can be directed to Melinda Cullins (mcullins@sulross.edu).

FOR GRADUATION:

1.  Complete an Intention to Graduate form (available through Tomas Hernandez (tohernandez@midland.edu) and Brandi Bell (bbell@midland.edu)).

2.  As soon as your report becomes available from the respective SRSU Dean's Office, IMMEDIATELY consult with Midland or SRSU agents if any discrepancies are noted.

3.  Be mindful of deadlines!  May graduation 2007 regular deadline is Friday, September 29th, 2006.  Chat with Kendall Craig if you have questions (kcraig@sulross.edu).
