Registration Forms

For a student club to be considered a registered student organization, paperwork must be filed in the Campus Activities Office at the beginning of the school year. These forms include contact information as well as club bank account permissions and signatures. To register an organization, print, complete and submit the documents below:

Once a club has submitted completed forms to the Campus Activities Office, it will be considered an officially-registered club at SRSU. This allows clubs to legally raise funds on campus; to be included in official publications such as recruiting pamphlets and campaigns, student life brochures, etc.; to use SRSU facilities at no cost; and to advertise meetings and other club events on campus.
