Meningococcal immunization is mandated by state law for new and transfer students under 22 years of age who are on campus. Student must be vaccinated against meningococcal no later than ten days before the semester begins.

Where do I send my vaccination records?  

Forms can be submitted and uploaded through your Student Profile on go.sulross.edu (Admissions Personal URL).

*All proof of meningitis vaccines uploaded to your Student Profile (Admissions Personal URL) must be clear, easy to read and include the following: Name, birthdate, type of vaccination received, and Date the vaccine was received

Vaccine conscientious objection policy as of October 1, 2013

The only means by which a student can object to vaccination for enrollment purposes is to use the DSHS’ current, official form for conscientious objection.  This form can be requested online and takes 5-7 business days to arrive by mail. [STUDENT MUST SIGN THEIR OWN DSHS AFFIDAVIT FORM, NOT A PARENT].

Affidavits must be notarized and submitted to the Student Health Services in University Center room 211, or mail to: Health Service Coordinator, Sul Ross State University, PO Box C-60, Alpine, Texas 79832.

Proof of Meningococcal vaccination or waiver must be submitted via the Student Profile of their PURL. Contact your enrollment services representative if you have questions. Immunization data is stored for 10 years from the date of receipt or last vaccination entry, whichever is later. Regular treatment notes are only kept for seven years. All expired documents are properly destroyed.

Students entering animal sciences or health care need to contact their program director about additional immunizations and/or boosters that may be required. Students with a chronic medical condition, older than 49 years of age or Native American should check with their health care provider for vaccine recommendations.
